Adult Carer Support Plans – What you can expect
An Adult Carer Support Plan is an important way of identifying your needs as an unpaid carer.
One of our Carer Link Workers (CLWs) will arrange a good time to meet you to have an informal discussion about your caring role. You’ll discuss the kind of caring you are carrying out, how many hours care you provide, other caring services that might be involved, if you have a Self Directed Support budget, other relevant agencies you might be in touch with, how you are feeling, what you are doing to give yourself some ‘time out’ and how caring is affecting your life. At the moment these sessions will take place using Zoom,telephone or NHS Near Me due to the ongoing Covid 19 situation to protect both carers and staff from infection risk.
Connecting Carers are introducing access to NHS Near me for appointments. Carers can now use the secure video calling platform when arranging appointments with Connecting Carers. If this is something you would be interested in, please discuss with your Carer Link Worker
You could also discuss other things that you might want to consider as an unpaid carer; do you need Power of Attorney or Emergency Planning, would a short break help you recharge your batteries, is there a support group nearby you could join?
The whole conversation is about making sure you are as informed and supported as you want to be. We can help you to find out if you are receiving everything you are entitled to and support you as a carer. It is important to look after yourself too so our Carer Link Workers will give you some time to think about what would help you to ensure you still have a life outside of caring too.
